PGLO - Manager of Programs and Partnerships
PGLO is seeking an experienced knowledge translation expert, with a deep understanding of the needs of older adults in Ontario and the health and social care workforce to join our dynamic team as the new Manager, Programs and Partnerships.
The Manager, Programs and Partnerships will oversee the development, implementation, delivery, coordination and evaluation of PGLO’s capacity building, education and research programs catering to older adults, care partners, and health professionals focused on the care of older adults. Reporting to the Executive Director and working in collaboration with the PGLO staff and facilitator team, this role leads all aspects of program development and supports key partnerships within and external to PGLO. With a focus on curating engaging and informative content for a wide variety of audiences and fostering collaborations, the Manager, Programs and Partnerships ensures the delivery of high-quality educational experiences for various audiences and supports the PGLO team to build and maintain effect partnerships across Ontario.

Career Opportunity - Nurse Practitioner - Medical Program & Specialized Geriatric Services
North York General Hospital is seeking a Nurse Practitioner
Nurse Practitioner (NP) Medical Program The Nurse Practitioner (NP) is a registered nurse in the extended class (EC) with the College of Nurses of Ontario (CNO) who has obtained additional education (Master’s degree). NPs have a legislated scope of practice that is beyond that of a Registered Nurse in the general class: the authority for NPs to initiate and perform controlled acts is subject to the limits and conditions outlined in the Nursing Act (1991) and the CNO standards of practice. The Nurse Practitioner demonstrates advanced knowledge, skill, and judgment in providing care to patients that is consistent with current College of Nurses of Ontario Practice Standards for Nurse Practitioners and the current competencies defined by the Canadian Nurses Association Competencies for Nurse Practitioners. As an NP you will provide specialized and advanced nursing care for the in-patients within the Medical program and Specialized Geriatric Services. You will collaborate with the interprofessional team and most responsible provider to help patients and their families’ transition across the health care continuum. You will be expected to perform comprehensive nursing assessments and geriatrics consultations for patients with complex and challenging health care needs across programs. You will assume a leadership role and collaborate with the broader health care team and community partners in managing complex and challenging patient care situations. In this role you will work in partnership with program leadership and front-line staff to develop, implement and evaluate best practices in geriatrics care across the organization. You will initiate and participate in professional and organizational activities to advance nursing practice and facilitate improved patient care
outcomes.

The CoP is hosted by our talented team here at the RGP of Toronto. Representatives from OHTs in the GTA who join the CoP will be able to create connections with other teams focusing on older adult populations, hear stories of successful integrated care models, and exchange knowledge on implementation strategies. During and in-between meetings, our team provides CoP members with coaching and training on how to implement and scale successful frailty care pathways and models. Updates on events, resources, and services
As of March 16, 2020 all our in-person conferences or educational events have either been postponed, cancelled, or converted to virtual meetings. Sign up for our webinar mailing list to learn more about virtual events, including training for health workers redeployed to long-term care.
Misinformation and information overload have been key challenges during the current COVID-19 pandemic. One of the ways we have been supporting healthcare providers during this challenging time is by curating and creating COVID-19 resources that are specific to the care of older adults with frailty. We have been sharing these resources with our network through our "RGP COVID-19 Updates" (approximately weekly). We also have a COVID-19 resource page.
