Support for Ontario Health Teams

The greater Toronto area (GTA) Ontario Health Teams (OHT) Community of Practice (CoP) on Senior Friendly Care is a mechanism for OHTs to create connections with other teams focusing on older adult populations, hear stories of successful integrated care models, exchange knowledge on implementation strategies, and receive coaching from experts in seniors care.

Senior Friendly Care

The goal of Senior Friendly Care (sfCare) is for healthcare providers in all sectors to work collaboratively with each other and with older adults and their caregivers to achieve the best possible outcomes for older adults. The sfCare Framework serves as the foundation for what senior friendly care should look like, and the sfCare Getting Started Toolkit provides a self-assessment tool and report card, implementation resources, and training modules.

Senior Friendly Hospitals

As part of our work on the pan-LHIN Senior Friendly Hospital (SFH) strategy, we led the launch of SFH ACTION (Accelerating Change Together in Ontario) in 2015; a complex knowledge-to-practice (KTP) initiative. We continue to support this collaborative of 87 hospitals with 400+ participants, who are working together and sharing their successes in quality improvements that focus on senior-friendly care for hospitalized older adults.
