Assessment and Management Tool – SF7 Nutrition Toolkit

The SF7 Toolkit supports clinical best practices for healthcare providers across the sectors of care and includes self-management tools for older adults and their caregivers. The toolkit provides a common practice framework that complements the unique skills and practices of the various care providers helping older adults. SF7 focuses on seven clinical areas that support resilience, independence, and quality of life: cognition (particularly delirium), mobility, social engagement, continence, pain, nutrition, and polypharmacy.

Other – sfCare Learning Series for Clinicians: Nutrition Poster (multi language)

Co-developed with Canadian geriatric clinical experts, the sfCare Learning Series for Clinicians provides the following resources on 7 key clinical topics: delirium, mobility, polypharmacy, pain, loneliness, nutrition, and urinary incontinence:

  • Introductory Clinical Education Modules – a PowerPoint presentation with accompanying speakers notes, which can be self-directed learning, or facilitated in a group setting.
  • Posters – intended to be displayed in a healthcare environment, and are designed to stimulate conversation between a patient/caregiver and healthcare providers.
  • Handouts – intended to be used by clinicians, including geriatric specialists, during care conversations to support patient and caregiver engagement and patient self-management. For additional support, clinicians may also recommend the sfCare Learning Series for Caregivers which comprises a website, handbook, and online course.
