The sfCare Self Assessment Tool is part of the sfCare Getting Started Toolkit. The toolkit includes tips on how to prepare for the self assessment, such as printing the questions to aid in collecting the information.

The sfCare Self Assessment is designed to identify your organization's strengths and opportunities. The results of your assessment will be reviewed and a report card will be sent to the email address provided.
